
So tired...man i feel dog tired... swam an hour, had a whole day of running around waking patients up and making them feel like lab rats. Tried to be nice about it but man, honestly i am still after the juicy signs that i can elicit! And now... 920pm... i got 3 hours left to study.Welcome to NUS Medicine... wonderful curriculum.
Attended the combined teaching on acute abdomen and the only thing i learnt is that there is a lot to learn. And if some1 came to me with a life-threatening condition, with a hole somewhere in his viscus or is bleeding so much that his abdomen fills with blood, the best thing i can do is to not send him for CT scan. Why? Cos there is no room in there to do any resuscitation.
So how? Do an x-ray, hopefully pick up gas under the diaphragm or funny looking loops of bowel and quickly dump him on a surgeon who would supposedly know what to do. Preferably do this in the middle of the night, just so that you can enjoy that moment of being able to tell some1 more senior than you to do something that he really doesn't want to do. heh....
